I have found that having the questions in front of me while
I'm organizing a field trip helps. That way I won't have a
zillion different phone calls to the organizer on the other end.
This is the form I use.
We love to take pictures and share them with our friends. I
have this form available on all the field trips I plan so as to
respect the families who do not wish to have their pictures
displayed publicly.
By attending the field trip you agree to waive all liability for damaged, lost or stolen items on the above mentioned field trip against Bev Mazurowski or designated point person, ___no one on this field trip___. You also agree not to hold these persons responsible for injuries caused by lack of parental supervision, facility default or negligence. Any actions causing negligence, damages or injury to another person or facility is the responsibility of the guardian and/or parent. The persons named above are not to be held liable. You also understand that as an open homeschool group we do not require back ground checks on people in attendance, other groups or individuals who may be present or on staff at the facility where the field trip is. Bev Mazurowski or the above mentioned point person cannot be held responsible for any actions or lack of action that would result in a civil or judicial lawsuit.

We are fortunate enough to have so many homeschoolers in our
area who attend many field trips. Sadly enough we cannot vouch
for everyone on staff at a facility or the other folks who attend (it
doesn't even have to be a part of our own group). For liability
protection, anyone who attends the field trips I organize signs in on
a roster with the note you see below attached. I provide copies to
anyone who wishes to take one. I also add it to the web pages I
build and anywhere else it seems relevant.
Sign In Rosters help keep a head count of who is there, who
has paid, etc.
To avoid overbooking a field trip I keep a running roster on my desk top.
Here's a sample of one.
I also save all emails in a separate folder and save the email addresses in
my contact list with any necessary information. If you use yahoo email you
have a choice to put these contacts in multiple categories. I make one up
for each field trip. So if I want to send email to everyone signed up, I just
need to go to contact, click the category and click send mail.
It's nice if you send a confirmation back to the person rsvp'ing confirming
the totals, dates & time, just to make sure everyone is on the same page.
